We are looking for a Patient Flow Co-ordinator to join our Team based in Coventry. The Urgent and Reablement Service is fast paced, diverse and covers a wide range of specialities supporting the multicultural Coventry Community.

These are new roles designed to be dynamic and exciting. You will be responsible for managing all waiting lists on a daily basis and identifying fluctuations and rationale for this. The role will include working with clinicians to facilitate the coordination and allocation of work to the HCA staffing group, facilitating appropriate packages of care to meet the patient’s needs, with an aim to prevent avoidable hospital admissions
You will support patient movement across the wider system partners, working collaboratively with the Urgent Care & Reablement team as well as external agencies across health and social care services, to ensure joined up patient centred care is delivered within agreed timescales.
You will keep IT systems updated with regard to patient data. You will prepare reports and data with a focus on length of stay. You will support and lead on projects relating to patient flow looking in detail at capacity and demand models.
