We are delighted to be working with our clinical colleagues in Primary Care Networks and GP Practices to ensure that patients receive a better experience; having direct and speedy access to mental health care specialists as well as Navigators to help with a range of social issues; including housing, debt, isolation, poor wellbeing, relationship issues.

We are very pleased to be collaborating with our partners, Living Well Consortium and Better Pathways to deliver a high quality and equitable service.

The Service

The main aim of the Primary Care Mental Health Navigator Team is to work as part of a collaborative partnership with both Forward Thinking Birmingham (FTB) staff and Birmingham and Solihull Mental Health Foundation Trust (BSMHFT) staff within primary care setting(s). This service is an integral part of local Primary Care Networks (PCN) across Birmingham.

The service will see a team of Mental Health professionals, Local Authority, Education and voluntary sector staff working together to deliver speedy and place-based interventions and support to patients in their community. The service is an all-age service and will be based in a primary care setting.

The Role

  • The Primary Care Navigators will work in a team providing support to patients who are experiencing poor mental health and wellbeing. You will work collaboratively with clinical staff and patients to support their recovery; action planning and signposting to relevant agencies.

  • You will input activity and report against key performance indicators and outcomes in line with service level agreements and requirements from the Senior Recovery Navigators and Service Manager.

  • You will work jointly with partners at The Living Well Consortium and Better Pathways. Attend team meetings, service reviews, individual supervisions, and appraisals.

  • You will network and liaise with community assets to grow the range and number of support agencies/organisations to ensure that each area is represented, and patients have choice.
