A great opportunity has arisen for a registered health care professional to work as a Clinical Operations Manager within the professional and highly dedicated community nursing and therapy team, based in Aylsham.

Our patch includes the market town of North Walsham and the seaside town and village of Cromer and Mundesley. This is a beautiful part of rural and coastal North Norfolk with many tourist attractions nearby.

You would be responsible for providing clear operational leadership and line management to NN2 community team, which consists of nursing and therapy clinicians, APs, HCAs and phlebotomists. You will participate in the leadership of the wider community nursing and therapy teams within the North Place portfolio. You will manage the mobile workforce and take responsibility for monitoring lone working and staff adherence to policy. You will take responsibility for the data quality monitoring of the team and will ensure that Trust annual priorities and quality goals are embedded within the workforce.

Key Areas of Responsibility
  • To provide clear operational leadership and line management to the community teams and clinic based services within the locality portfolio.
  • To facilitate delivery of service specifications
  • To ensure services are able to provide in-reach to inpatient units and procured beds where required
  • To work with all health care professionals and statutory/non statutory agencies to provide a seamless, integrated service to our service users
  • To oversee that clinical supervision is occurring within the team and accurately recorded.
Main Responsibilities

1. To relay information from the Head of Operations I respect of NCH&C and local issues to all members of the teams on a regular basis in the form of business meetings and discussing relevant issues relating to the delivery of clinical services
2. To ensue patients are provided with appropriate interventions to enable them to remain within their own home, where possible avoiding inappropriate admissions
3. To participate in the staff appraisal scheme as both an appraise and appraiser, maintaining a dynamic personal development plan
4. Be aware of the Trust’s Behaviour Framework and ensure these are embedded in professional practice
5. To undertake the measurement and evaluation of the team’s work and current practices through the use of evidence based practice projects, audits and outcome measures and lead others in the evaluation of the work
6. Make recommendations for lead and change
7. To communicate at different and at times complex levels to meet needs of GP’s, staff and patients, using tact and diplomacy
8. To actively contribute to the modernization an forward movement of NCH&C, being actively involved in and taking an interest in changes within the NHS, the potential impact if local and national policies and the implications for the service
9. To demonstrate a sound understanding of clinical governance and risk management, and apply to work situation and ensure the team do. To identify, assess and document the risk, identifying, assessing and documenting risk if implementing change
10. To facilitate development of the integrated team and individual staff members to promote job satisfaction, performance and an ability to manage change
11. To be responsible for the day to day operational line management of the teams. To plan, co-ordinate and evaluate the services to ensure they are safe, efficient and effective with regard to patient management and use of staff time
12. To actively perform absence management, submitting forms in a timely manner, as per the Trust’s policy, working closely with HR
13. To be responsible for the compliance of mandatory training and appraisals within the team, ensuring KPIs are achieved
14. To be an authorized signatory for goods, services and payroll
15. To accurately complete personal human resources documentation in respect of team members. Ensure that designated time frames are adhered to.
16. Complete all data returns as required by NCH&C. Collate, report, manage and analyse information on activity levels and quality, in accordance with agreed performance standards
17. To develop and initiate undertaking of audit of the service, evaluating he results and formulating an action plan in respect to service developments and quality standards. Take part in and carry out audit as required i.e. Patient record audit
18. To identify clinical and organisational improvements to enhance and improve the integrated are and services to patients, implementing change and leading others
19. To assist in the marketing and promotion of the service with fellow practitioners and stakeholders
20. To monitor and manage the team budget and assist the Head of Integrated Care to effectively manage resources
21. To monitor and manage staff absence, annual leave and study leave in line with NCH&Cs policies, procedures and guidelines. Inform the Head of integrated Care of any significant trends/ issues
22. To be actively involved with disciplinary and complaints procedures, where appropriate, and managing investigations of incidents
23. To comply with existing organizational ad departmental policies, procedures and guidelines and to be actively involved in the reviewing of and updating of them appropriate. Ensure policies are interpreted with support from Human Resources and that designated staff implement policy and service developments
24. To propose and implement departmental policy changes fir the management of patients referred to the service, networking with other specialist practitioners
25. To represent the locality at strategic meetings with the CCGs when requested
26. Participate in the development of business cases
27. Project management, including CQUINN initiatives
28. To operate a clinical capacity model in order to predict and act according to changes in service demand, ensuring the effective use of resources including the reallocation of staff

Clinical responsibilities

1. To offer advice and support to other members of the primary healthcare team
2. To provide a mentorship role to team members
3. To be involved in the provision of relevant professional advice and expertise in matters regarding the service
4. To keep accurate, confidential and up to date records for all patient contacts
5. Ensure activity data and clinical information is recorded in the patient held record (where available) and on SystmOne and completed on a daily basis
6. To be familiar with and comply with all relevant measures under the GDPR and Caldecott Guardian initiative, vulnerable adult policies (including safeguarding) and the NCH&C clinical governance framework
7. To attend regular supervision meetings with a nominated supervisor and provide supervision for other team members
8. To participate in continuing professional development activity and all mandatory training as required
9. To practice within NM/ COT/ CSP professional codes of conduct and protocols
10. To promote the effective use of local services by closer networking with other providers of health, social care and voluntary services

Hours of Operation

The service operates between 8 am and 6 pm seven days a week and you will be required to work shifts between these times.

You may be required to work a 24 hour shift pattern in the future, should this service be commissioned by the Trust.
