We have a fabulous opportunity for a Band 6 nurse and would welcome applications from a Registered Mental Health Nurse.
  • The post-holder of the role will support and work proactively to facilitate patient flow through the team to improve clinical pathways, efficiencies and patient outcomes within the role’s remit through an integrated system approach
  • To facilitate bed allocation for all specialties by ensuring that patients are placed into the most appropriate Discharge to Assess bed that reflects their need promptly and in doing so support to meet national target
  • To support the system of pull within the organisation and system, engaging with all areas that impact patient flow across as required
  • To facilitate effective discharge planning
  • To ensure the smooth and timely transfer of patients
  • To have an ongoing awareness of patient flow issues g. bed availability, current bed state, outliers and bed closure
  • To work with the Hospital Discharge Team to develop expertise and effective high quality patient flow systems
  • To work within the agreed protocols to liaise with other organisations and specifically for the repatriation of patients to ensure efficient utilisation of beds.
  • To ensure data collection for audit trails thereby ensuring effective utilisation of beds and relevant data quality standards are maintained in line with NHSEI
  • To be responsible for the booking and tracking of admissions with the relevant to the Continuing Healthcare Process
  • To ensure all actual and predicted discharges are reported on a daily basis and to inform Discharge Lead Nurse
  • Understand the discharge lounge role and importance of lounge for patient.
  • Oversee repatriation and ensure there is a completed written summary of actions undertaken and ensure swift repatriation of patients to other healthcare settings.
  • To have regard for patients’ cultural and religious
  • To collect and present other data when needed to meet monitoring targets
  • Point of contact for barriers to effective patient flow, taking a proactive lead role with the team to manage the situation and escalate accordingly

  • Identify the needs of patients in connection with their admission
  • To work collaboratively with other navigators / patient flow managers to ensure appropriate and accurate information is exchanged in a timely manner
  • Support the development of joint multi-agency protocols and work plans to ensure improvements are mutually agreed, tracked and escalated
  • Nurture key relationships and maintain networks internally and externally, including
  • Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care
  • To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes and delivery plans as required to facilitate others learning.
